Reimagine Care works with healthcare providers and insurance plans to coordinate home-based care, reducing the need for unnecessary inpatient visits. Reimagine’s clinicians remotely monitor patients, collaborate with oncologists to assess additional service needs, and deploy home care workers as required.
The partnership between Memorial Hermann and Reimagine Care began last year, with Memorial Hermann also announcing an investment in Reimagine Care.
This partnership grants cancer patients 24/7 access to a specialized oncology team. Patients can connect with a team member via SMS, video, or phone to receive educational content focused on oncology, address concerns about symptoms, or access at-home care if necessary.

THE LARGER TREND
Earlier this year, Reimagine received an investment from Oncology Ventures, a venture capital fund focused on cancer, though the amount was undisclosed.
Reimagine Care also partnered with Michiana Hematology Oncology (MHO) in Northern Indiana to extend cancer care services beyond clinic walls.
Last year, Reimagine Care teamed up with Denver-based DispatchHealth to offer oncology patients access to virtual-to-home care, ensuring coordinated care between Reimagine Care, DispatchHealth, and the patient’s primary oncology team.
Additionally, Reimagine Care partnered with the Center for Cancer & Blood Disorders in Dallas-Fort Worth to provide a home-based supportive care and symptom management program.
In 2022, Reimagine secured $25 million in Series A funding and announced a partnership with Memora Health, a complex care management platform. Reimagine would utilize Memora’s clinical intelligence platform to expand its virtual care center, supporting cancer patients and caregivers at home.
Reimagine Care has also collaborated with the University of Colorado Anschutz Medical Campus to care for bone marrow transplant patients and with California-based City of Hope.
